Currently Unique Items do not have Unique in-game models, but this will change.
How to Obtain a Unique Item
Random Drops - Uniques can be found as random drops just as any other item. The actual drop rates are unknown to us and the only info we have to go on is:
Orb of Chance - There is a very small chance of upgrading a normal item into a unique of the same base type when using an Orb of Chance. For example if you want the Andvarius unique ring then you should use an Orb of Chance on a normal(white) rarity Gold Ring. Success rates for Orbs of Chance are unknown but the chance to create a unique should be about the same chance as finding one in the wild.
Event Prizes - Occasionally, unique items are offered as top tier prizes in special Event Competitions. Please note that not all events offer unique items as prizes.
Unique Mods
Most unique mods and their values are static. Occasionally, as seen with the x% Increased Evasion Rating on Hyrri's Ire, the mods can have a variable value range and are able to be rerolled using a Divine Orb. Multiple copies are needed of each item to confirm which values are static and which are variable.
Also, some mods can be found elsewhere in the game through skill gems or passives, but not on any other equipment. Some mod values can only be obtained on uniques even if the mod itself is not naturally unique. For example, the Mana Leech mod is normally 1% on any given item but can be 5% on Ephemeral Edge. Additionally, mods like the 20% Increased Physical Damage on Ezomyte Peak, are indeed unique because IPD is not normally available on a helmet.
All mods that include "when on Low Life" are activated when Life drops below 35%.
