This quest is given upon discovering the entrance of Vaal Ruins Level 1 in the River Crossings. To complete this quest, you must first complete The Weaver's Needle and Delving into Sin.
Once you have the Wicked Spear and the Baleful Gem in you possession, the roots preventing access to Vaal Ruins will be cleared allowing you to progress further.
